چکیده

CASE REPORT A 79-year-old woman sustained a left anterior column acetabulum fracture after falling on ice. On initial presentation to the emergency room she was placed in femoral pin traction. The reduction of the acetabulum in traction was not concentric and the femoral head began to migrate centrally. Treatment options, including primary THA after the acetabulum fracture healed, were discussed with the patient. The traction was discontinued at 6 weeks, and the patient was allowed to transfer from the bed to the chair. Three weeks later she was ambulatory with a walker, nonweight bearing on the affected side. When the acetabulum fracture healed, she was advanced to weight bearing as tolerated. No further migration of the femoral head was noted (Figure 1). Surgery was initially planned for when the fracture healed, but at the patient’s request was postponed until 6 months after the initial injury. At surgery, the patient was placed in the lateral decubitus position and a posterior approach

Acetabular Fractures in the Elderly

ABSTR ACT: As the population ages, the incidence of osteoporotic fractures, including those of the pelvis and acetabulum, continues to rise. Treatment of the elder patients with an acetabular fracture is much more controversial than the treatment of younger patients with similar injuries, where prevention of posttraumatic arthritis and total hip replacement remains optimal to limit need for rev...

Reconstruction of the Acetabulum in Developmental Dysplasia of the Hip in Total Hip Replacement

Developmental dysplasia of the hip (DDH) or congenital hip dysplasia (CDH) is the most prevalent developmental childhood hip disorder. It includes a wide spectrum of hip abnormalities ranging from dysplasia to subluxation and complete dislocation of the hip joint. The natural history of neglected DDH in adults is highly variable. The mean age of onset of symptoms is 34.5 years for dysplastic DD...

Acetabular Fracture after Hip Hemiarthroplasty: One Stage Procedure to a Total Hip Arthroplasty after Stabilization of the Fracture by Means of Cerclage Wires

Introduction Hip fractures in the elderly pose an increasing problem in society. In the elderly, a bipolar hemiarthroplasty (HA) remains the treatment of choice in case of hip fractures related to osteoporosis. However, due to an ongoing osteoporosis in this age group, a periprosthetic femoral fracture and a fracture of the unresurfaced acetabulum are increasingly noticed. In the literature, no ...

Acetabular Fractures in the Elderly: Midterm Outcomes of Column Stabilisation and Primary Arthroplasty

Background. Interest in arthroplasty techniques for periarticular or intra-articular fractures in the elderly/osteoporotic patient continues to rise, including for geriatric acetabular fractures. In line with this, many acetabular fracture surgeons are now undertaking acute total hip arthroplasty in elderly/osteoporotic patients. Little is known however of the outcomes of this procedure, beyond...

Pelvic Discontinuity Caused by Acetabular Overreaming during Primary Total Hip Arthroplasty

Intraoperative acetabular fracture is a rare complication of primary total hip arthroplasty (THA), typically occurring during impaction of the cementless acetabular component. Here we report an unusual case of pelvic discontinuity caused by overreaming of the acetabulum during primary THA. Restoration of posterior columnar continuity was achieved with an autologous fibular graft and a reconstru...
